Admin And Customer Service - Nfp Organisation

Be Recruitment are recruiting a Care Coordinator to provide advice, guidance and service coordination to aged care, disability, mental health consumers.

The Care Coordinator will provide responsive and individualised services to each participant within the organisation's policy and will arrange and monitor services to fulfil support plans.

Review intake information on client database and discuss as required
Develop, coordinate and deliver the support plan for clients, organise services and liaise with service providers regarding quality delivery and client needs
Facilitate the development of the clients support networks and community engagement
Refer clients to relevant supports and external service providers as required
Liaise with agencies and service providers on behalf of the client.

This role is working from home 3 days per week and in the office in Bella Vista 2 days.

Ability to prioritise to meet deadlines
A commitment to person-centred practice and maximizing the opportunities and support of participants.
A sound understanding of relevant legislation and full range of community services that participants may require.

Ideally you will have the following or be willing to obtain:
Current Australian driver’s licence
Current First Aid and CPR Certificate
Willingness to undergo a Police Check
Hold an NDIS Workers Screening Check
Hold an employee Working With Children's check
